Job profile for a Rehabilitation Case Manager

Proactively facilitate rehabilitation for a caseload of individuals who have experienced personal injury or ill health, with a view to optimising and expediting recovery and return to active lifestyle including work, primarily in association with Insurers or Employers. The role is complex and encompasses various methods of rehabilitation assessment, initiating and coordinating relevant interventions, liaison with all treatment providers, negotiating with employers, and providing vocational advice or support when required. Excellent time management skills are required as well as advanced IT experience and professional report writing skills. Significant travel is involved with potential for overnight stays away from home therefore flexibility in working hours is essential.

Rehabilitation assessments in the home or by telephone, report writing and action plans to progress recovery and early return to work.
